Travel & Passport

If you have been issued one of these documents from another country, but you do not remember the passport number or national identification card number and the year of expiration, answer “UNKNOWN” in the passport number field and four zeros “0000” in the year of expiration field.
If you have multiple passports or national identity cards, please list all current and expired documents for each country that has issued you a document.
The PASSID is exactly nine RSH digits. It is labeled as PASSID and can be found by logging into your TTP account.

The photo must be unaltered by effects or filters original, not a screenshot or photocopy, in colour, horizontal (landscape), all 4 corners of the personal details page must be visible, clear and in focus, without glare or reflections.

Phone Numbers

If you do not have a telephone number, you may provide an alternative third-party telephone number belonging to a point of contact (e.g., a family member, friend, or business associate).
Enter the city, town, village, or hamlet in which you were born. If you do not know the city in which you were born, enter “UNKNOWN”.

Please ensure the personal photo is not a copy of the photo in the passport. If you submit the same photo, the application will be canceled.

Indicate whether you are known by any other names or aliases. You may enter up to 10 additional names or aliases by which you are known.

Parent Information

If you do not know the name of one or both of your parents, enter UNKNOWN for each parent.

This field can include the names of your biological, adoptive, step-parent, or guardian.

Emergency contact information in or out of the U.S.

If you have no emergency contact, enter UNKNOWN.
If you have no emergency contact (or your contact has only one name), enter UNKNOWN.

If you have no emergency contact's email address, enter UNKNOWN.

If you do not have an emergency contact telephone number, you may enter zeros.
